PREREQUISITE: Basic Life Support (BLS) CBT is required to be completed within 90 days of attending a hands-on skill evaluation course. 1. Current BLS Provider Certificate: All BLS instructor candidates are required to have a current BLS provider-level certification. Red Cross and the Military Training Network (MTN) limit Training Sites to a specific instructor to instructor trainer ratio (15:1). To continue supporting personnel outside of the 375 MDG, and maintain successful program, we need a proactive instructor cadre.

You will be attending a week of Mercy Hospital Rotation. Please make sure you do not schedule appointments, training, leave or anything that will keep you away from your rotation. Mercy Hospital Training Affiliation Agreement (Labs Tests Training Validation Sheet)
----Labs-visit Immunizations/Public Health review and initial—Flu vaccine w/in 12 mos, COVID Vaccination required (Immunizations fill out dates), Hep B Vaccine-3 doses required. Request a copy of your Immunizations Record. ***TB test-A NEGATIVE PPD or TB quantiferon blood test w/in past 12 mos f/AF Medical Personnel if member has been deployed overseas w/in past 36 mos or if member has been in contact w/a person infected w/TB in last 12 mos. All other members-NO TB exposure in 12 mos and NO DEPLOYMENT in past 36 mos will require a NEGATIVE PPD or TB quantiferon blood test w/in past 36 months. See sheet required items for Positive TB tests. (validated by Immunizations/Lab)
----HIPAA-needs to be done within last 12 months and not expire while in Mercy Rotation (provide copy of certificate when submitting packet)
----Life Support (BLS/CPR card)-expires every 2 years. Make sure it is not expired or will expire during your Mercy Rotation (provide copy of card/certificate when submitting packet)
----First Day Orientation- To be completed by Education and Training
----Rotation Preference- You will be able to train in certain units to fulfill 4N0X1/46N3 CMRP Checklist tasks. Units are: Adult ED, Peds ED, Trauma and Intensive ICU, and Burn Unit. Categorize them where you would like to go first, second, back up 1 and back up 2. (This way we know in what order to make your rotation).

The Mercy rotations are specifically set up to complete the 40 hr (4N0)/144 hr (46N) requirement of ER or ICU experience. You may work whatever shifts you like, but make sure you complete all your scheduled hours. Refer to the Rotation Expectations for shift hours/times. You may see many other items on your CMRP checklist during your time at Mercy but you will not be able to have experience with all of them. For example, Mercy does not use REBOA, TXA, or the MACE exam, and none of the deployed environment section will be covered. Most of these are knowledge based training and can be completed by viewing the resources or links located in the CMRP checklist under training source. Make sure you are aware that Mercy is not a “one and done” for CMRPs.
**This rotation will help for your annual CMRPs. You might not be able to do everything on that checklist, but you need to be involved as much as possible and accomplish the items needed. Make sure you have the CMRP Checklist and focus on those tasks needed. **
Meal Card Holders/gas mileage: Those on meal card can file a missed meal voucher. This can only be completed until after your rotation is done. You would also have to keep track on what meals you missed in order to accurately get them reimbursed. Please go to the First Sergeant about this since the missed meals have to be documented through him and the Commander endorses as the verifier OR work with the DFAC to have a boxed lunch for pick-up while doing the rotation. You might need to have someone in your chain make the request for you, but it at least gives you a good lunch option. Unfortunately gas mileage is not reimbursable since Mercy Hospital is considered to be within the local area (within 100 mile radius).

You will be attending a week of Mercy Hospital Rotation. Please make sure you do not schedule appointments, training, leave or anything that will keep you away from your rotation. Mercy Hospital Training Affiliation Agreement (Labs Tests Training Validation Sheet)
----Labs-visit Immunizations/Public Health review and initial—Flu vaccine w/in 12 mos, COVID Vaccination required (Immunizations fill out dates), Hep B Vaccine-3 doses required. Request a copy of your Immunizations Record. ***TB test-A NEGATIVE PPD or TB quantiferon blood test w/in past 12 mos f/AF Medical Personnel if member has been deployed overseas w/in past 36 mos or if member has been in contact w/a person infected w/TB in last 12 mos. All other members-NO TB exposure in 12 mos and NO DEPLOYMENT in past 36 mos will require a NEGATIVE PPD or TB quantiferon blood test w/in past 36 months. See sheet required items for Positive TB tests. (validated by Immunizations/Lab)
----HIPAA-needs to be done within last 12 months and not expire while in Mercy Rotation (provide copy of certificate when submitting packet)
----Life Support (BLS/CPR card)-expires every 2 years. Make sure it is not expired or will expire during your Mercy Rotation (provide copy of card/certificate when submitting packet)
----First Day Orientation- To be completed by Education and Training
----Rotation Preference- You will be able to train in certain units to fulfill 4N0X1/46N3 CMRP Checklist tasks. Units are: Adult ED, Peds ED, Trauma and Intensive ICU, and Burn Unit. Categorize them where you would like to go first, second, back up 1 and back up 2. (This way we know in what order to make your rotation).
